Patients use Nuna daily, insights are shared with Hill’s Population Health Team.
Which of my patients are eligible?
Eligible patients are select Hill Network Medicare Advantage Patients who struggle with medication adherence and have one or more chronic conditions. (hypertension, diabetes, hyperlipidemia).
To find out if which of your patients are eligible please reach out to the Hill Population Health Team.
What do I need as a Clinician?
Nothing! Nuna is designed to be low-lift for clinicians. Hill’s Population Health Team may contact you if a patient has a concerning trend.
With Nuna you can:
- Encourage patients to enroll if they have been notified they are eligible.
- Request Individual Patient activity data
- Reinforce Nuna’s benefits during encounters
Nuna does not replace clinical workflows or require new documentation tasks. It simply helps patients stay on-track between visits.
