What is Nuna and how does it benefit patients?
Nuna is a mobile app designed to support patients in managing chronic conditions between visits. It helps patients stay engaged in their care by making it easier to track blood pressure and medications, receive timely reminders, and know when to seek additional support. The goal is to support patients in following their care plan and staying connected to their care team.

Which patients are included?
Aetna Medicare Advantage patients on the hypertension registry with a recent blood pressure reading and an active MyChart account. We estimate approximately 8,000 Cleveland Clinic patients across Ohio meet these criteria.

Do I need anything?
No. There is no expectation for providers or care teams to enroll patients or change current workflows. If patients mention Nuna, you may:
- Encourage app enrollment if they were notified they are eligible
- Reinforce Nuna’s benefits
- Encourage tracking of clinical data (BP, meds)
