If a patient submits an abnormal reading, they receive an in-app message explaining that the care team is being notified. Patients are also instructed to call 911 or go to the nearest emergency room if their symptoms worsen or they believe they are experiencing a medical emergency. Nurse follow-up and any additional outreach are handled according to established escalation protocols.
Patients should be directed to the MyChart message or the contact listed in the communication. Providers may also reference the provider landing page for additional context.
The pilot will run for three months. Feedback will be gathered during and after the pilot to help determine next steps.
