Welcome to Nuna
You’ve been confirmed as eligible to enroll in the ACCESS Model through Nuna Health. This page explains what enrolling means, what you can expect, and what rights you have. Please read it before completing your enrollment.
Enrollment is your choice
Enrolling with Nuna through the ACCESS Model is entirely voluntary. Choosing not to enroll will not affect your Medicare coverage or your ability to receive care from any provider.
No cost to you
Your cost-sharing for ACCESS Model services through Nuna is $0 per month for the duration of your enrollment.
One provider at a time, per clinical track
While enrolled with Nuna, only Nuna may provide ACCESS Model services to you for each clinical track in which you are enrolled. You may, however, be enrolled in more than one ACCESS clinical track at the same time, with Nuna or with different providers, if you have qualifying conditions in multiple tracks. You may continue to see any doctor or provider of your choice for all other care.
You can leave after 90 days
You may end your enrollment with Nuna and enroll with a different ACCESS Model provider at any time after 90 days from your enrollment date. To unenroll, go to Settings in the app or email app-support@nuna.com.
Your Medicare claims data
To support your care over time, Nuna may request access to your Medicare claims data from CMS. If we do, your Medicare claims history may be shared with Nuna for health care operations purposes, subject to all applicable federal privacy and security protections including HIPAA.
By enrolling, you are giving your consent to that data sharing if and when Nuna requests it. You have the right to modify your data sharing preferences at any time by calling 1-800-MEDICARE (1-800-633-4227). Please note that even if you choose to limit data sharing with Nuna, CMS may still engage in certain limited data sharing for care coordination, quality improvement, and population health activities.

Sharing updates with your care team
As part of the ACCESS Model, Nuna will proactively share Care Updates, summaries of your health status and care activity, with your Identified Care Team (the other doctors and providers involved in your care). By enrolling, you consent to Nuna sharing these updates with your care team. If you prefer that Nuna not proactively share Care Updates with your other providers, you may decline by contacting us at app-support@nuna.com.
About the research study
The ACCESS Model is a federal payment model test conducted by CMS. As part of this study, a small number of individuals are randomly assigned to a comparison (“control”) group by CMS. If you are assigned to the control group, you would not be eligible to enroll in the ACCESS Model with any participating provider for 12 months. This assignment is random, is not based on anything about you or your health, and does not affect your regular Medicare benefits, rights, or coverage in any way. While unlikely once you have been confirmed eligible, this determination is made at the time of enrollment.
What you are agreeing to
By completing enrollment, you confirm that you understand and agree to the following:
- You are choosing to participate in the ACCESS Model through Nuna, and you understand that participation is voluntary.
- Your cost-sharing for ACCESS Model services through Nuna is $0 per month.
- While enrolled, only Nuna may provide ACCESS Model services to you for each clinical track in which you are enrolled. You may be enrolled in multiple ACCESS clinical tracks simultaneously.
- You may end your enrollment or switch to a different ACCESS Model provider at any time after 90 days from your enrollment date.
- Your Medicare claims data may be shared with Nuna for health care operations purposes, subject to federal privacy and security protections including HIPAA.
- You consent to Nuna proactively sharing Care Updates with your Identified Care Team. You may withdraw this consent at any time by contacting app-support@nuna.com.
- There is a small chance you could be randomly assigned to a study control group at the time of enrollment, which would mean you are not eligible to enroll for 12 months. This will not affect your regular Medicare benefits, rights, or coverage in any way.
