Welcome to Nuna
You’ve been confirmed as eligible to enroll in the ACCESS Model through Nuna. This page explains what enrolling means, what you can expect, and what rights you have. Please read it before completing your enrollment.
Enrollment is your choice
Enrolling with Nuna through the ACCESS Model is entirely voluntary. Choosing not to enroll will not affect your Medicare coverage or your ability to receive care from any provider.
One provider at a time, per condition
While enrolled with Nuna, only Nuna may provide ACCESS Model services to you for your enrolled condition. You remain completely free to see any doctor or provider you choose for all other care, this enrollment does not limit your freedom to choose your healthcare providers.
You can leave after 90 days
You may end your enrollment with Nuna and enroll with a different ACCESS Model provider if you’d like at any time after 90 days from your enrollment date. To unenroll, [contact us / use the account settings in the app]. The process to leave is no more complicated than the process to join.
Your Medicare claims data
To support your care over time, Nuna may request access to your Medicare claims data from CMS. If we do, your Medicare claims history may be shared with Nuna for health care operations purposes, subject to all applicable federal privacy and security protections including HIPAA. By enrolling, you are giving your consent to that data sharing if and when Nuna requests it.
You have the right to modify your data sharing preferences at any time by calling 1-800-MEDICARE (1-800-633-4227). Please note that even if you choose to limit data sharing with Nuna, CMS may still engage in certain limited data sharing for care coordination, quality improvement, and population health activities.

What you are agreeing to
By completing enrollment, you confirm that you understand and agree to the following:
- You are choosing to participate in the ACCESS Model through Nuna, and you understand that participation is voluntary.
- While enrolled, only Nuna may provide ACCESS Model services to you for your enrolled condition.
- You may end your enrollment or switch to a different ACCESS Model provider at any time after 90 days from your enrollment date.
- There’s a small chance you could be assigned to a study “control” group, which would mean you’re not eligible to enroll for 12 months — this won’t affect your regular Medicare benefits, rights, or coverage in any way.
- In the future, your Medicare claims data may be shared with Nuna for health care operations purposes, subject to federal privacy and security protections including HIPAA.
