A mobile app to help your patients

What is Nuna

Nuna is a mobile app that helps your patients manage chronic conditions (hypertension and diabetes), adhere to medications, and develop healthy habits. Patients can use Nuna to track vitals, blood glucose levels, medications, and exercise.

Data is shared real-time with the Hill Physicians Population Health Team. When clinically appropriate, patient outreach is conducted by Hill Physicians’ pharmacists or health educators to provide education and coaching.

Which of my patients are eligible?

Select Hill Physicians patients were invited to enroll in the Nuna app for medication adherence and/or disease management support.

What do I need to do as a Clinician?

Nothing! Nuna is designed to be low lift for clinicians. The Hill Physicians Population Health Team will notify you if a patient has a concerning trend.

If a patient asks you about Nuna:

• Encourage app enrollment if they have been notified that they are eligible.
• Reinforce Nuna’s benefits.
• Encourage tracking of clinical data through the app.
